Objective: This study aims to deeply analyze the space construction techniques of gasless endoscopic thyroidectomy via the axillary approach and, combined with personal practical experience, to enhance the standardization of the surgical process, accelerate the acquisition of surgical skills, and reduce the risk of surgical complications. Methods: This study adopted a retrospective analysis method to comprehensively explore 36 patients who underwent gasless endoscopic thyroidectomy via the axillary approach at our hospital between August 2018 and January 2023. We meticulously recorded the clinical outcomes, potential complications, and cosmetic results of the surgeries. Results: All 36 surgeries were successfully completed without the need for conversion to open surgery. Although the surgical duration ranged from 1.5 to 2.5 hours, slightly longer than that of traditional open surgery, the notable cosmetic effects and the absence of major complications fully demonstrated the advantages of this surgical approach. Conclusion: This study emphasizes that profound anatomical knowledge and exquisite endoscopic skills play a decisive role in successfully constructing the surgical space. By sharing surgical insights and techniques, we hope to effectively shorten the learning curve for doctors, reduce surgical complications, and thereby enhance the overall safety and efficacy of the surgery.
